The bank said the system failure began at 8:40 a.m. and was resolved by 10:07 p.m. It said there is no evidence indicating a cyberattack.

Clearly this was another case of software project managers not filling out enough excel sheets to checkboxes to ensure a high level of quality.

Therefore, the answer is clearly to add another excel sheet with additional checkboxes so as to prevent recurrences.
