As usual, America is exceptional but in all the wrong ways.

So the ideal is having none ? The inhabitants have certainly a good deal. But that big village uses the trained doctors, the facilities for complex treatments, the medicine supply and research from other countries. That makes big savings for the State. Plus they sabotage the health of neighbors with their discounted cigarettes.
It's hardly more developped. Brunei, Macao, Singapore. Then Monaco and Lichtenstein ? That really takes all value of the article that they compare artificially rich free cities will countries with large populations and diversified economy.

Healthcare can be the best in the world - but it means nothing if it is way too expensive for the ordinary folks.

I can vouch for USA being a healthcare mess. The richest third world country in the world for sure. I'll take Japan health care any day.

The world's best countries for socio-economic development have small populations, according to several polls, it should be noted. Bigger ain't better.
