So I guess it's harmful in the same way any outdoors sport that requires you to keep track of an item is harmful. Twisted ankles! Oh, no!

I plan to play this game when it hits the Japanese App Store... but I honestly don't have any sympathy for people getting hurt while not paying attention.
As fun as it looks, it's just going to continue to grow the zombie like crowds of people in Shinjuku station that have no idea they are blocking your path while playing cell phone games and chatting on Line.
