I've seen video and this was quite comical in a "the joys of being old" way. Koike was quite ambitious and actually lifted her knee like a real pitcher to get her bodyweight into the throw. Most teen idols just stand still and throw a mega loopy ball using their arm alone, which results in the ball barely reaching the plate.

I had a similar injury in my ankle-heel when I was in my late 20’s. So I can say that it does also happen to young people. And I was definitely not physically unfit at the time.

Basically, when a load bearing joint is hyperextended, the base of the tendon tears off the bone, taking a piece of the bone with it.

Without proper examination by Xray or CT it can be misdiagnosed as a sprain, which was almost my case.

My son torn a knee ligament while just going after tennis balls. He has fully recovered after a month naturally.

I got my knee ligaments overstreched but that was after a ski fall on a black slope. Did go back all down the slope on one knee unaided. Recovered over a year naturally but could do every sports (marathon, badminton...) outside knee strong rotating like Koike did.

A fracture is severe and probably due to age.

I wish her a quick recovery and pretty sure she will never attempt that again, from experience.
