Get some insights about the real sources of pollution.

Simple. The source is pretty much humans and our activity.

Bizarre to think I've been in Japan for twenty years and I have never seen Fuji san. Never really had the compulsion. Am I missing much?

Perhaps too many. I'd love to see a bigger variety of photos from around Japan. JT, why not ask readers to submit photos for display? The BBC does this on their site, and they can be very good. Below is a link to an example of readers' photos from Scotland.

There are 24 hilly streets within the 23 wards of Tokyo named “Fujimi Zaka”, of which only 4 currently have any view of the mountain due to new construction.

Go to the OK supermarket (足立小台店 Adachi Kodai branch) rooftop parking lot located in the river bank between Sumida and Arakawa rivers to have the best unblocked Tokyo view ever, including the great Fujisan, 2 Tokyo towers, Shinjuku, Ikebukuro, Saitama and Yokohama.

I'm always amazed how Fuji-san can seem so close but in reality is so far away. I once had a very clear view from central Ibaraki.

OK photography folks, what kind of lens/equipment would be necessary to get this kind of shot?

No trickery required just a lens with very long focal length. The photographer would have be a long distance from the airport building to achieve this effect.

Yep looks like some post processing of the image. According to Google maps Mt. Fuji is about 95km (60mi) away.

We often see pictures like this where it looks like Fuji-san is right next to Tokyo landmarks, but obviously that's not the case. I'm guessing most people can't take this picture on their smartphone cameras.
