The emperor is not a "world leader." He holds no political position in Japan. The heads of government of other nations can choose to bow to him observing Japanese custom or not.

Nothing, but he's not held in particularly high regard. He will probably be shown the respect and deference generally due to a head of state, and one day he might be held in high regard if he carries on like his father has.
