It is common at his age for the first sign of heart disease to be a heart attack and death, unless he is quickly treated by someone who recognizes the symptoms.

A few friends died in their 40s from heart attacks.

One of my business partners felt really bad during a trip overseas, in BsAs and effectively stayed in bed for the last 4 days before heading home. At home, he got bypass surgery and started taking fitness seriously. That was about 10 yrs ago and he's healthier now than ever before, just by consistently walking to get over 8K steps in a day.

And two recent studies published in JAMA Neurology and JAMA Internal Medicine this September, investigated the connection between daily step count, intensity of steps and an increased health benefit.

That link has more details about risk reductions for steps and speed. The study had 75,000 participants in the UK. Walking faster is important. 112 steps per minute people reduced their risks of dementia 38% in the study, in addition to the other health benefits for the 80+ steps per minute group. Don't mosey.

Not necessarily, respiratory tract infections can trigger heart attacks with some frequency, specially with other risk factors like overworking. The death is for sure a sad tragedy, but not necessarily a product of incompetence or malice.

He felt pressure in his upper chest, a classic indication of an oncoming heart attack. I know because it happened to me, and a proper diagnosis kept me alive. Feel sorry for his wife and friends.
