In this case, "alert" means conscious and responsive to others. It's a good thing.
Unfortunately this kind of incident is an unavoidable part of baseball- there's really no way around it without putting a permanent screen in front of the pitcher, which would cause all kinds of havoc with batted balls.
A few years ago, after another particularly ugly incident, there was some talk of using special padded caps to protect pitchers, but nothing came of it. The padded caps were particularly unattractive as I recall, which I'm sure didn't help things.

Pitchers need better protection. They could just make them wear batting helmets for a start. Glad he's responsive and will hopefully be back to his old self on the mound and at home.

When I played baseball, pitching always had me nervous because I worried about something similar happening. I hope Tanaka recovers smoothly.

what is alert meant to mean?

Move the mound back, make the ball softer, give the pitcher a hard hat. Honestly, I think baseball is one of the safest sports out there. I'm more concerned with the amount of pitches in Japanese High School baseball.
