Not a US State, its citizens cannot vote in the US. Presidential/Congressional elections.
It's an unincorporated territory controlled by the US.

Many people think Puerto Rico is a country. It’s not. It’s an unincorporated territory of the US. They are US nationals.

Congratulations to the runner from Puerto Rico. It is refreshing to read a story about athletes from an imperfect nation that is nonetheless proud of her country.
