Read again. The father is in prison and there`s no mention of whether the grandfather still has anything to do with the family business or problems with the law.

A mafia Family does not refer only to biological relatives, such as uncle, brother, dad, etc. Family indicates anyone who is a member of a particular gang. And I doubt the police hands are to small. Rather most mafias are well run, have high paid attorneys and know how to circumvent the system. That is until their enemies blow them away...
