Well, when the prefecture pretty much doesn't test at all, like five or ten per day for 1.4M people, and the US forces in Okinawa test at 100 per day for about 45K... In fact, even if you're sick, the prefecture hospital won't test you unless you have a link to being in mainland Japan in the last week... The Americans on Okinawa have a two-week quarantine upon arriving on the island, and test anyone with a fever or a sniffle. What's the local government doing here, other than hiding cases in order to get those potentially infected tourists from mainland to come down? What if the American Marines caught it from the local population, which is the most likely scenario?
A doctor at the prefectural hospital went around the government and told the population they would test 100 people who simply wanted to be tested, on the basis that they were in or around American Village on the 4th of July. Why didn't the local government do this?
